Dedicated to the memory of Helen McCrory OBE who played Polly Gray in Series 1-5 it says in the closing credits. Apparently she passed away last year at the age of 52 due to the effects of cancer. McCrory had a daughter and a son with her husband Damian Lewis, best known for the "Band of Brothers" and "Homeland" series.
